Domperidone
(
Suppositories )

Composition:
Each suppository contains:
Domperidone...................................10mg,30mg or 60 mg.
 
Properties
Domperidone is selective peripheral dopamine antagonist in chemoreceptor trigger zone (CTZ )It selectively blocks peripheral dopamine receptors in the GIT wall, thus enhancing normal synchronized GI peristalisis & motility in the proximal portion of the GIT, it may also counteract anticholenergic- induced relaxation of the lower esophageal sphincter.

 

Pharmacokinetics
Peak plasma levels are achieved within 1 hour after rectal administration. Its half life is about 7.5 hours.About 90% is bound to plasma proteins.It is metabolized in liver & eliminated in urine & faces.

 

Indication
Short-term treatment of nausea & vomiting of various origins (functional, infectious, organic or dietetic & those induced by radiotherapy or drug therapy )

 

Side effects
Plasma prolactin concentration may be increased which may lead to galactorrhoea or gynaecomastia.
Reduced libido.
central effects such as extrapyramial dystonic reactions or drowsiness.
Occasional rashes and other allergic reactions.

 

Contraindications
Domperidone is contra indicated in patients with known hypersensitivity to this drug, or when stimulation of gastric motility may be dangerous e.g. : G.I.T hemorrhage, obstruction or perforation.

 

Drug interactions
Opioid analgesics & antimuscarinics may antagonise the prokinetic effects of domperidone.
Domperidone may antagonize the hypoprolactinaemic effect of drugs such as bromocriptine

 

Precautions
The dose frequency should be reduced in patients with renal insufficiency.
Use in children is restricted mainly to nausea and vomiting following cancer treatment.
In treatment of nausea and vomiting in Parkinsonian patients, therapy may be continued for maximum 12 weeks.
Drug is avoided during pregnancy and lactation.

 

Dosage
Adults:30-60 mg every 4-8 hours
Children: ( over 2 years )
Body weight 10_15kg :15mg twice daily Body weight 15.5_25kg :30mg twice daily
Body weight 25.5_35kg :3 times daily Body weight 35.5_45 kg :4 times daily

 N.B: supp. may be cut into 1/2 for younger children
 

Storage
Keep at temperature not exceeding 15°C.
Keep out of reach of children.
